Case Management is a program designed to assist members with significant health risks or medical concerns by identifying, implementing and monitoring the services required to assist them in achieving the best possible outcome. It also plays a vital role in the management of chronic or catastrophic illness or injury. The Case Management program provides support and assistance by Certified Case Managers(CCM) to patients, families, physicians, facilities and other healthcare providers involved in the patient’s care.
